CLAUDINE THOMAS
In the mid 1970’s Claudine Thomas was in graduate school and made a decision to focus on services to medically fragile and older adults with developmental disabilities. She worked for many years as a Social Worker and Executive in developing and implementing service delivery systems in various locations in the United States. The need to develop residential alternatives, day programs and transportation systems was essential to meet the overwhelming demands of placing individuals being discharged from state facilities and mental hospitals.

Claudine abides by the philosophy of providing the most appropriate services in the least restrictive setting for each of the hundreds of individuals she has served. If it is the desire of the Elder and Family for the Elder to remain in their own home, a detailed supportive services plan in developed and implemented. If it is the desire or need to secure an Assisted Living Facility or Skilled Nursing Facility, the choices are made with respect and dignity. Claudine has worked extensively with families making difficult decisions regarding residential placement, medical interventions and end of life decisions. Anticipatory grief and after loss counseling are offered with sensitivity, dignity and respect.

Claudine became a member of the National Association of Professional Geriatric Care Managers in November, 2006 and is a member of the Western Region of Geriatric Care Managers. She serves individuals who reside in Ventura and Los Angeles Counties and has helped with transferring and relocating clients to be closer to their loved ones. Claudine helps out-of-area families with assessments, resource development, and crises intervention.
