A Caring Heart For Seniors is a Professional Geriatric Care Management Company
that offers a variety services to elderly persons and their family members.
Services are individualized and specific to a time and place in a person’s
life. All services are offered with compassion, empathy and understanding and
available 24 hours a day when needed. Following are some of the excellent services
offered:
1. Professional assessments of cognition, medical, mental health, depression, safety, risk of falling, financial, social, legal, spiritual, self care abilities, and need for support. Development and implementation of a person-centered Care Plan based on individual assessment and findings. Reassessments offered as needed.
2. Effective communication, collaboration and coordination with the elder, family members, care giving agency, home health specialists, doctors and other professionals and providers.
3. Resource development, referrals and information dissemination on seminars, support groups, bill-paying services, conservatorship, durable medical equipment, free services, entitlements, lectures, law changes, benefit updates, social gatherings, classes, clubs, and specialists in various areas.
4. Consumer advocacy, education and interventions with difficult behaviors, Alzheimer’s Disease, Parkinson’s Disease, Pick’s Disease, Lewy body Dementia and other age-related or health-related problems.
5. Cost containment which helps to avoid inappropriate choices of services, placement, duplication of services and unnecessary treatments.
6. Quality control by adherence to the National Association of Professional Geriatric Care Management’s Standards of Practice and Pledge of Ethics as well as National Association of Social Workers Code of Ethics.
7. Screening, arranging and monitoring for the highest quality of in-home caregivers, respite services, nurses, nutrition services, therapies, equipment, hospice care and other services.
8. Monitoring quality of services in residential facilities and assessing
appropriateness of level of care.
9. Crises intervention, emergency preparedness and disaster planning.
10. Transportation to and/or accompanying an elder to a medical appointment and communicating with family members via email or telephone on results and findings.
11. Counseling, support and stress management for caregivers to support their own health and well-being.
12. Conflict resolution, family mediation, and facilitating communication within families.
